Know your rights before services begin
Illinois rules require licensed providers to give clients and patients information about their rights. These include respectful treatment, information for informed participation, confidentiality protections, and a complaint process. Ask for the complete written policy before consent.
Discuss consent and information sharing
Ask what the proposed service involves, who provides it, and what information may be shared. A court referral is not a reason to skip questions about confidentiality or releases. The applicable policy should explain permissions and exceptions.
Ask how to raise a concern
Request the grievance process and relevant oversight contact information. Keep a copy of any concern you submit and the response. Discuss communication accommodations so you can understand the process.
Keep useful questions available
Bring these questions to the first appointment.
- May I have the rights, consent, and privacy documents?
- How are records protected and copies requested?
- Who may receive information about my participation?
- How can I question a recommendation or raise a complaint?
- What communication or accessibility arrangements are available?
An educational overview
This is not a formal Notice of Privacy Practices, legal advice, or certification that a particular privacy regime applies to this preview. Amazing Grace DUI’s finalized consent forms, privacy policies, and grievance contacts must be supplied and reviewed before services or live intake are offered through the site.
