Start with the request

Identify the authority asking for information. Court requirements and the Secretary of State driving-relief process are not the same. A checklist on this website is a preparation aid, not a substitute for your written instructions.

Common records to discuss

Not every document below applies to every person. Confirm the exact list with the evaluator, treatment provider, or agency.

  • Alcohol and Drug Evaluation Uniform Report and any updates.
  • Risk-education completion certificate.
  • Early-intervention completion and follow-up records, if applicable.
  • Treatment plan, discharge summary, and continuing-care plan when treatment was recommended.
  • A continuing-care status report, when requested.
  • Court orders, hearing notices, and agency correspondence.

Check report dates and signatures

Ask whether the report is current for its intended use and who may supply an update. Keep complete copies without adding handwritten changes to a signed Uniform Report. Additional information should be handled by the evaluator through the appropriate documentation process.

Request copies through the right channel

Contact the provider for clinical and completion records. The Secretary of State has a separate process for copies of administrative-hearing documents. Ask how identity and authorization are verified before records are released.

Do not enter arrest narratives, medical details, or identification numbers in the website preview. Arrange a secure record-sharing method directly with the intended recipient.
