Start with your specific request

Use these answers to prepare. Your written evaluation and instructions from the court or agency determine what you need; a general website answer cannot decide your personal requirements.

18 questions shown

Do I need an appointment?

Yes. Amazing Grace DUI is by appointment only. Call 773-255-8139 to request and confirm your appointment before visiting 1200 North Ashland Ave, Suite 510, Chicago, IL 60622.

Does this website send my appointment request?

When online requests are available, review your contact details and select Send appointment request. The request is emailed to the appointment team for follow-up. The form states when delivery is unavailable or limited to local testing. No visit is reserved until the team confirms it.

Is Amazing Grace DUI licensed or court approved?

Licensure, credentials, and court-acceptance details have not been supplied for this website. Confirm those details with Amazing Grace DUI and your referring authority before enrolling. The site does not claim statewide court approval.

Is an evaluation the same as a class?

No. A DUI evaluation produces findings and recommendations. Education, intervention, and treatment are different services. Start with the written recommendation and ask which service is required.

What should I bring for an evaluation?

Ask for the provider’s specific list. Have your ID, referral, available driving and arrest records, chemical-test or refusal information, and previous evaluation or service records available. Arrange a secure sharing method.

What is needed before risk-education enrollment?

Illinois requires a completed evaluation recommending risk education. Confirm identification, schedule, fee, and attendance expectations before enrollment.

Are online classes allowed?

Illinois permits online DUI risk education subject to its requirements. Confirm the provider, format, and acceptance for your case. Amazing Grace DUI’s online availability has not been verified for this website.

How long is DUI risk education?

The Illinois minimum is 10 instructional hours over at least four different days, with no session longer than three hours. The provider should supply the exact schedule.

How do I receive a completion certificate?

Discuss attendance and testing requirements with the provider. Ask how the certificate is delivered and whether you or the provider must send it to the court or agency.

Can I choose my risk classification online?

No. An evaluator determines the classification from the assessment and required records. This site’s classification guide explains categories but is not a self-assessment tool.

Are the minimum hours my entire requirement?

Not necessarily. The written recommendation, clinical needs, subsequent information, and court or agency instructions may require more than the initial minimum. Review the complete plan with the provider.

Does an older evaluation need an update?

For an alcohol- or drug-related Secretary of State hearing, an update is required if the last Uniform Report is more than six months old at the hearing. Confirm who may supply the update for your classification.

What records might a hearing require?

Depending on the recommendation, records may include the evaluation, course certificate, intervention documents, treatment plan, discharge summary, continuing-care plan, and status report. Confirm the exact requirements with the agency.

What do services cost?

A verified Amazing Grace DUI fee schedule has not been provided. The rates page lists the categories to ask about. No other provider’s prices are presented as Amazing Grace DUI prices.

Is insurance or a payment plan available?

Insurance acceptance, assistance, and payment arrangements have not been confirmed. Ask directly about the specific service before assuming coverage or making payment.

Can a service satisfy an out-of-state DUI requirement?

That depends on the authority handling the matter. Confirm the provider, report, course, and format the other state accepts. An Illinois service should not be assumed to satisfy another state’s instructions.

Can I upload records or describe my case here?

No. Do not enter sensitive case, medical, or identity information in the appointment form. Ask the provider for a secure record-sharing method.

Does completing services restore my license?

Completing recommended services is separate from the decision about driving privileges. Confirm the remaining requirements with the Illinois Secretary of State or other responsible authority.
