Have the evaluation ready
Illinois requires a completed evaluation recommending risk education before enrollment. Review your signed Uniform Report and ask whether any additional documents are needed. Do not assume a class registration can replace the evaluation.
Your enrollment checklist
Keep these items available for a direct conversation with the provider. This website does not accept uploaded legal or health records.
- Your completed evaluation and written referral or order.
- A current photo ID.
- The court or agency name and any written deadline.
- Questions about attendance format and accessibility needs.
- A request for the current fee schedule and cancellation policy.
- A secure method for sharing any required documents.
Confirm the schedule and course requirements
Ask for the dates, attendance rules, test requirements, and completion procedure before paying or planning travel. Review additional recommendations separately; early intervention or treatment is not automatically included in risk education.

Use the appointment-request form when it is available, or call 773-255-8139. The team must confirm your appointment separately. Sending a request does not enroll you in a class, reserve a place, or take payment.
