For alcohol- and drug-related Illinois Secretary of State hearings, an updated evaluation is required when the last Uniform Report is more than six months old at the hearing. Who may complete the update varies by classification and agency history.
Gather the prior documents
Bring the original evaluation and your most recent recommendations. Discuss any services completed since that report and any changes in circumstances.
- Original Uniform Report and previous updates.
- Risk-education and intervention completion records.
- Treatment, discharge, and continuing-care documentation when applicable.
- The hearing date and any agency letters.
Confirm who can complete the update
Do not book with a new provider solely because the appointment is convenient. Ask whether the original evaluator, the treatment provider, or a properly documented file transfer is required in your situation.
