A plan for what comes next
Continuing care connects the end of active treatment with ongoing support. Illinois calls for patient input and a plan addressing recovery support, prevention of recurrent problematic use, and any applicable monitoring responsibilities.
Make the plan practical
Discuss the activities that fit your circumstances, how often they should occur, and how you will know when additional help is needed. Plans should be individualized rather than copied from a standard number of meetings.
- Recovery-support activities and helpful community connections.
- Appointments or referrals that support ongoing needs.
- Steps for seeking additional care if problems return.
- How to document participation when a court or agency requires it.
Keep your records together
Ask for a copy of your discharge summary and continuing-care plan. For a driving-relief hearing, review the Secretary of State checklist with the provider to identify any additional status report.
Talk about changes early
Bring questions about scheduling, barriers, or changing needs to your provider. Completing an initial service is a transition point, not a reason to ignore new concerns.
